DEVELOPMENTAL DISABILITIES SERVICES

NY Residential
PSCH offers a variety of residential options in NYC.  PSCH strives to maintain a family atmosphere within the residences.  The group residential opportunities offered include Independent Residential Alternatives (IRA) and the Intermediate Care Facilities (ICF).  Great care is taken to ensure that individuals are appropriately placed with the proper combination of residents and staff within each program.

NY Day Programs
PSCH currently offers five day program options within the borough of Queens.  Located in Flushing and  Astoria, the STATION ROAD and ASTORIA DAY Treatment Programs assist individuals working on activities of daily living that include meal preparation, money management skills, and basic health and hygiene, while integrating community involvement and volunteer experiences in a traditional day treatment setting.

The 117 St. Day Treatment Program offers an experience similar to Station Road and Astoria Programs.  However, this program is geared towards the special needs of the medically frail and aging population.

The Long Island City Day Habilitation Program allows for daily volunteer experience with organizations such as Salvation Army, SNAP, a program which assists senior citizens with shopping and Meals on Wheels.

Day Habilitation Without Walls is an innovative program with a hub-site in Flushing.  The goal of this program is to provide immersion in the community through volunteer experiences.  The Participants in this program also work toward developing computer skills, proper interview techniques and resume writing, with the long term goal of obtaining competitive employment.


PSCH New Jersey

PSCH currently operates seven residences in New Jersey.  There are 36 residential opportunities in Morris, Burgen, and Passaic counties. The Administrative office is located in Hackensack and will also serve as the hub-site of the Day-Habilitation Program.

Clinical Services
PSCH offers a variety of Clinical Services throughout its Development Disabilities Services continuum.  These services include Physical Therapy, Occupational Therapy, Speech Therapy, Social Work, Nutritional Counseling ,Nursing and Psychological Services.

 

 

 

 

 

NY Family Support Services
Family support services offers assistance to individuals who still reside at home with their families.

With Medicaid Service Coordination (MSC), individuals are visited at their home monthly.  These programs ensure that the individuals can continue to live at home in a safe and healthy manner, by providing advocacy, linkages and referrals to services.  PSCH currently employs five bilingual MSC's thereby serving a great need within communities.

The At Home Residential Habilitation program sends workers in to assist community individuals in order to attain a maximum level of independence.  The participants work on a variety of person-centered goals including travel-training , cooking and community integration.

The Family Reimbursement allows families to submit receipts annually for the reimbursement of  approved participant items , such as  furniture, camp, or clothing.

PSCH's Family Support Services department also offers the option of Individual Support and Services (ISS) grants to assist individuals and families in the community.  The financial assistance offered through this program is often the key to allowing an individual to remain at home with their parent or family.
