MENTAL HEALTH SERVICES

PSCH Provides services to adults with a history of a serious mental illness.  Furthermore consumers may have a co-occurring substance history and/or a history of homelessness.  We offer a host of comprehensive services such as residential, treatment, case management and vocational.  All programs are for adults 18 and over.

Residential Services
This Program provides a housing continuum where individuals receive appropriate levels of supervision and services to develop skills, achieve personal goals, increase independence and thrive in the community.  Specialized services are provided for individuals with mental illness and chemical addiction (MICA).


Community Residence/Single Room Occupancy
This Queens Residence consists of 44 furnished studio apartments with private bath and kitchenette.  Tenants are responsible for the maintenance of their own apartment and are able to live fairly independently with minimal supervision.  Services are provided to enhance their self-sufficiency and daily living skills with the goal of more independent living.

Apartment Treatment Program
The Apartment Treatment Program consists of apartments in Queens and Brooklyn.  Shared living arrangements enable up to three consumers to live together with a degree of independence while continuing to have supported services.  Case managers visit on a regular schedule, assisting consumers to develop or strengthen daily skills, manage medications, shop and prepare meals, handle crises, budgeting and personal relationships.  Services assist consumers to cope, function and thrive in a community setting.

Supported Housing
Located in Queens, Brooklyn and the Bronx, our supported housing consists of single and shared apartments which provide permanent independent housing.  The consumers have demonstrated a significant period of psychiatric stability and have shown the ability to live in the community with minimal case management assistance.  Tenants pay 30% of their income towards rent.


Project Clean
Project Clean is a VESID sponsored program designed to train individuals with psychiatric disabilities in janitorial maintenance and landscaping.  There are training sites in Queens, Brooklyn and Manhattan.


I CAN
I CAN is a VESID sponsored program designed for individuals interested in exploring and/or pursing various careers.  The program empowers consumers to use their experience, education and interests to identify and access employment in a field of their choice.  They are provided with internship opportunities to further help them gain competitive employment. 

Supported Employment
Supported Employment is a VESID and OMH sponsored program designed to provide intensive job coaching services to consumers who have begun working in the community.  The goal is to provide long term support thereby helping the consumers maintain their employment.

                                                                                         

Blended Case Management
Our Queens and Brooklyn Blended Case management is a team approach model which combines the case loads of two Intensive Case managers (ICM) and one Supportive Case Manager (SCM). Blended Case Management Team provides a one-to-one support system which serves as an anchor for consumers as they navigate and progress through various systems.  The goal is to optimize the consumer's stability in the community and work with them in developing and utilizing support networks that will assist them in achieving and maintaining their functional goals.

Intensive Case Management
The Brooklyn program provides Intensive Case Management through weekly contact with their consumers.  The target population SMI (Seriously Mentally Ill) consumers who have been identified in need of more intensive services to maintain stability and function appropriately and effectively in the community.

Bridger Program

Based in Brooklyn, the Bridger Program provides transitional case management services to adults nearing discharge from the local State Psychiatric Center.  Services consist of individual and group counseling as well as case management to optimize the consumers placement choices in their discharge plans.

   
Assertive Community Treatment (ACT)
Assertive Community Treatment is a unique service for persons with mental illness in Brooklyn and Queens.  This innovative model provides a coordinated team of mental health professionals to assist each person with his/her specific needs relating to housing, medication management, life skills, substance abuse, socialization, family relationships and employment.  The program aims at reducing emergency room use and inpatient hospitalizations while supporting the consumer’s ability to live successfully in the community.
